Why people switch

Most switches happen for one of four reasons: needles, side effects, supply, or cost. Someone who has done well on an injection but dreads the weekly ritual may want a tablet. Someone who has plateaued on a tablet may want the stronger dual-receptor option. Neither direction is a failure, and neither is a step down — they are different molecules delivered in different ways, with different demands on your daily routine.

What matters is that the changeover is planned. Stopping one medicine and starting the other at a matching number is the single most common mistake, and it is the one most likely to produce a week of nausea or an unnecessary loss of appetite control.

The practical difference people underestimate is the timing rule. A weekly injection asks for thirty seconds of attention once a week. A daily tablet asks you to protect a thirty-minute window before your first food or drink of the day, every day. If your mornings are unpredictable, absorption becomes unpredictable too, and so do your results.

One point worth holding on to before you compare the numbers: the two medicines have never been tested against each other in a trial. The figures above come from separate studies in different groups of people over different lengths of time.

Switching from Mounjaro to the Wegovy pill

Tirzepatide has a half-life of around five days, so it clears slowly. A prescriber will normally have you take the first tablet seven days after your last injection — the day the next injection would have been due. This avoids stacking two appetite-suppressing medicines while also avoiding a gap long enough for hunger to come roaring back.

Even at the top of the injection ladder you are unlikely to start at the top of the tablet ladder. Oral semaglutide is absorbed erratically compared with an injection, and the gut needs time to adjust to a daily rather than weekly exposure. Building up over eight to sixteen weeks is normal.

Do not match the milligrams
The numbers on the two ladders are unrelated. 15mg of tirzepatide and 15mg of oral semaglutide are not comparable doses, and treating them as equivalent risks significant gastrointestinal side effects.

Switching from the Wegovy pill to Mounjaro

Going the other way is simpler, because oral semaglutide clears faster. The usual approach is to stop the tablets and take the first injection the following day, starting at 2.5mg — the standard tirzepatide starting dose — regardless of how high your tablet dose was.

  1. Take your final tablet as normal.
  2. Start Mounjaro 2.5mg the next day, and choose the weekday you want as your permanent injection day.
  3. Stay on 2.5mg for four weeks. This is a tolerance-building dose, not a treatment dose.
  4. Step up to 5mg at your next review if side effects are settled.
  5. Continue upwards no faster than every four weeks, guided by your clinician.

Some people worry that restarting at 2.5mg will undo their progress. In practice you are not starting from zero: appetite signalling stays partially suppressed through the changeover, and most people hold their weight steady during the first month rather than regaining.

What the first fortnight feels like

Appetite gets louder
Hunger often returns first in the evening. This is expected while the old medicine clears and the new one builds up.
Days 5-10
The new medicine takes hold
Nausea, if it appears at all, usually arrives here and is mild. Smaller meals and protein first make the biggest difference.
Days 10-14
Fullness returns at meals
Bowel habit often changes before appetite does. Keep fluids up and give it a few days before judging the dose.

Eat protein first, keep portions small, drink more water than feels necessary, and tell the clinical team if anything feels wrong.
Weigh weekly, not daily
Fluid shifts during a switch can move the scale by two or three pounds in either direction within days. A weekly weigh-in on the same morning tells you something useful; a daily one will only worry you.

When switching is not the answer

If your reason for switching is side effects, a dose reduction on your current medicine often solves the problem faster than a change of molecule. If your reason is a plateau, it is worth checking protein intake, alcohol, sleep and whether you are still in a deficit before assuming the medicine has stopped working. And if your reason is cost, say so plainly to your clinician — the dose you are on, not the medicine you are on, is usually the bigger lever.

Switching is also not appropriate if you have had a serious reaction to either drug class, if you are pregnant or trying to conceive, or if you have a history of pancreatitis or medullary thyroid carcinoma. Those are conversations to have before any changeover is planned.

Will I regain weight while switching?

Will side effects be worse after a switch?
